Medical Forms

To help expedite the registration process, the links below contain various forms that require completion prior to your visit and/or surgery. Please download the appropriate form as directed by your physician and/or staff and fill it out to the best of your ability. If you cannot access the form, please click on the Adobe Reader icon. This will allow you to download this software, which in turn provides access to the forms. Please bring the completed form(s) with you to your scheduled visit. Thank you.

Appointments

Please call to make appointments in advance. If you are unable to keep your appointment for any reason, please contact us as far in advance as possible so we may reschedule that time slot. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.

Preparing for Your Doctor Visit

New patients should arrive early to complete the registration process. In addition, please bring the following:

Driver’s License or State ID
Referral (If Required by Insurer)
Insurance Information
Copies of Operative or Surgical Records
X-rays, MRIs, and CT Scans from Prior Doctor Visits
A List of Current Medications and Any Known Allergies

Fees & Payments

At Quantum Orthopedics we make every effort to decrease the cost of your medical care. To do this, we request payment arrangements for all office services at the time they are rendered unless prior arrangement have been made. We accept cash, checks, MasterCard, Visa, and Discover. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient’s responsibility if the bill is not covered in full by your insurance. We will help in any way we can to assist you in handling claims.
